The goal of this study is to explore the adventures of owning and caring for pet gerbils. These survey questions were designed to achieve an understanding of the physical characteristics of gerbils and care of gerbils. You have been asked to participate in this survey, because you have some form of connection to gerbils, as a pet store manager, pet store worker, or a pet gerbil owner. As such, your insights provided on the survey will prove extremely valuable to the research study. It is estimated that completion of the survey will consume no more than twenty minutes of your time. Your name will not be used or in anyway associated with the responses of your survey as all participation will be strictly confidential. However, you may skip any questions that cause you to be uncomfortable, if you wish to do so. While participation in this study will not benefit you directly, participation in the survey will be essential to the research study.

I'm having a go at this now for you, but am finding it a bit awkward. There's a few things missing or just a bit off? The colour options are very precise and limited despite the amount of colours gerbils can be (so I'm using the 'other' option, not a huge problem as 'other' exists, but it'll probably make it a lot more work for your sis if everyone ends up using it :3 ). I have 5 gerbils so I had to just randomly assign 3 of them to answer questions about. On page 2 Question 2 '2. If your first gerbil has spots, where are the spots located?' There is no 'NA' option, probably an oversight? It's hard to answer questions based on your cage if you have more than one of different types and sizes and so on. Question 9 on page 2, it would be nice to have the temerature options translated into Celsius in brackets so that the survey answerers don't have to go and convert them themselves if they don't use Fahrenheit.
Just wanted to give some suggestions so I hope you don't take this as a criticism, I'll go convert the F to C now and continue answering questions ^^

A bit messy (repeating question) and not sure how to answer some, like if you have have two or more gerbils in a group when you buy them, does that count as first or first and second?
For size, do yo mean when bought? or when fully grown?
The room temperature will vary during the day by all those temperatures
The bedding is where they sleep, wood shaving are normally the substrate
